【摘要】 目的 检测血清CA72-4、CEA、AFP、CA-50、CA19-9含量水平，评价这五项肿瘤标记物对消化系统五种恶性肿瘤的诊断价值。方法 采用电化学发光免疫分析法测定经临床确诊的消化系统恶性肿瘤患者、消化道良性疾病患者和正常人血清CA72-4、CEA、AFP、CA-50、CA19-9含量。结果 五项肿瘤标志物在消化道五种恶性肿瘤患者中的含量均显著高于健康人群（P＜0.05）。 CA-50和 CA19-9在胆囊癌和胰腺癌两种恶性肿瘤与五项指标联合检测的检出率无显著差异（P>0.05）。CEA在直结肠癌与五项标志物联合检测的检出率也没有显著差异（P>0.05）。结论 不同的消化道恶性肿瘤引起血清CA72-4、CEA、AFP、CA-50、CA19-9含量水平不同。因此，针对不同的消化道恶性肿瘤应采用不同的指标联合检测。

THE DIAGNOSING VALUE OF TUMOR MARKERS IN SERUM FOR FIVE DIGESTIVE SYSTEM CANCER

Abstract Objective The aim of this study is to analyze the diagnosing value of five tumor markers including CA72-4, CEA, AFP, CA-50, and CA19-9 in digestive system cancer detected their level in serum. Methods Selected patients with digestive malignant and benign tumor and healthy persons, detected the level of CA72-4, CEA, AFP, CA-50, and CA19-9 in serum of them by electrochemiluminescence immune analyzer. Results The diagnosing value of five tumor markers with digestive system cancers were significant higher than in healthy persons (P＜0.05). The detecting ratio of cholecyst cancer and pancreas cancer by CA-50 and CA19-9 was shown same ratio with combined five tumor markers (P>0.05). The detecting ratio of rectum cancer and colon cancer by CEA were same as the ratio of combined five tumor markers (P>0.05). Conclusion Different digestive system cancer shows different level of CA72-4, CEA, AFP, CA-50, and CA19-9 in serum. So, when diagnosing different digestive system cancer, we should select different combination of tumor markers detection.

肿瘤标志物对肿瘤的辅助诊断价值已受到临床的广泛重视，但是肿瘤标志物用于人群肿瘤筛查却遇到了灵敏度不高、特异性不强、漏诊等问题［1，2］。联合检测有利于提高检测灵敏度。本文联合检测消化道恶性肿瘤患者、消化道良性疾病患者和正常人血清CA72-4、CEA、AFP、CA19-9和CA-50五项肿瘤标志物水平，分析及评价五项肿瘤标志物联合检测对消化道恶性肿瘤的筛查价值。
